Q. What makes your program unique?
A. A 30 day 100% guaranteed customer satisfaction, national coverage, and mail order contact lens
purchase program. No limitations or restrictions.
Q. Is coverage for individual or family?
A. The entire immediate family (spouse
and/or dependents) is covered by one single membership card that is issued to the member.
Q. What are the discounts on products offered
such as lenses, glasses, prescriptions, etc.?
A. Discounts are provided at the time of purchase. Members pay only the discounted amount
directly to provider. Discounts of up to 50% are provided on frames, lenses, and all optional items. The average savings is $72.00
per pair of single vision glasses. You may see discounts of over $100.00 on multi-focal lenses and frames.
Q. What discount is there
on Eye Exams?
A. OUTLOOK has over 10,500 optical providers. Many, but not all, provide 5% up to 50% off eye examinations. These providers
are in selected areas only, where approved.
Q. Are there any dispensing fees?
A. Most providers charge a nominal dispensing fee (less
than $10.00) at the time of purchase along with the discounted price for eyewear. This fee is seldom questioned by the member as the
savings will usually vary from $50.00 up to $200.00. The dispensing fee is a normal occurrence for participating professional providers
who are active in a PPO network.

A. OUTLOOK maintains a provider network of over
10,500 locations in all 50 States and Puerto
Rico. Approximately 86% of the U.S. population lives within a 15 mile radius of an OUTLOOK
provider.
